Tank overfill: when the environmental case sets the SIL

Two consequences from one cause. The fire case needs a risk reduction of only 5, below the SIL 1 range; the spill to drains needs SIL 1. And meeting the SIL 1 band is not the same as meeting the required risk reduction.

Scenario

A storage tank is filled from a transfer line. If the level control fails during filling, the tank overflows. Two consequences are assessed separately: a fire if vapour reaches an ignition source, and an environmental release if product reaches the drainage system.

LOPA: fire and safety consequence

Risk reduction needed: 5.0. That does not call for a SIL rated function.

LOPA: environmental consequence

Risk reduction needed: 50. This case sets the target: SIL 1, PFDavg ≤ 0.02.

Verification result

PFDavg 0.034 sits inside the SIL 1 band, but the environmental case needs a risk reduction of 50, which means a PFDavg of 0.02 or less.

Findings

Claiming SIL 1 because the PFDavg lands in the SIL 1 band would leave the environmental risk above the company criterion. The verification has to be checked against the required risk reduction from the LOPA, not only the band.

The valve dominates again, and its untested failures accumulate between replacements.

Recommendations

  1. Add a seat leak check to the valve proof test. Testing every six months without it is not enough (0.03); the leak check is what closes the gap.
  2. Keep the high-level alarm independent of the level control loop, or the LOPA credit for operator response is lost.
  3. Record as-found results so the coverage assumed here can be confirmed.
